So, I know this isn't right, but don't know what to replace to fix it.
Recoil plate is unattached to the recoil rod and I know it should be.
Doesn't slide onto it either as the end of the recoil rod is a little bit mushroomed preventing it.

Do I need a new rod and plate?

Here's the picture looking forward on the recoil plate (plate rotated to illustrate detachment and view of the rod end)

You probably need a new recoil plate if that's come off, and maybe a new recoil rod. The rear of the recoil rod is actually supposed to be "mushroomed", so it may be that your plate hole has gotten oversized. The rod enters the plate from the backside and then is attached at the front (bolt end) with a pin.

Check this page for more details: www.mac-11.info/bd.htm

You can get bolt parts by emailing vegaswbolts@gmail.com, or from Lage Manufacturing at www.max-11.com/M11Parts.html

Looks like the recoil rod has lost much of it "peen" complete the CF paper clip mod mac-11.info, dress up the rod end and proceed to rework the peen on the end. reassemble, better than new.
The ejector rod looks pretty ratty as well, rework that surface also.

Be careful that the spring doesn't come off and hit you in the eye! It looks compressed and barely held on by the peening...

That's exactly what I'd do right down to the paper clip mod. Medphys is correct. The recoil spring is under great tension, be careful when removing.

Shameless promotion...
I sell all the parts required to complete or rehab your bolt assembly, and I'd be happy to slot your recoil guide rod for the paper clip mod. http://www.uzitalk.com/forums/showthread.php?82630-WTS-CF-W-bolts-for-the-SWD-M-11-9

I would replace everything while you are at it.
Recoil spring
Recoil rod
Ejector rod
Recoil plate
Buffer

Look for quality parts. The are some low quality parts out there in the system.

Awesome, I'll go ahead and do the paperclip mod and dress up the peen on the end of the rod before I start shotgunning parts. Thanks guys!

For $30 it's not worth the effort. If the back plate and recoil rod are messed up odds are the ejector rod could be tweaked also. At the range in a bind sure, but the parts are so cheap there is no reason not to just put new ones in and know your gun is going to run IMO. Heck you can rehab the whole thing for less than $100. I didn't even shoot mine before replacing everything that touches the bolt. Why let a $10 part end your day at the range, but YMMV

Agreed. I'll pick up the bolt rebuild kit and just get it over with.
